These all sound like the problems associated with a horrible vacuum leak.

Also been told that possibly the choke is sticking on it.

This truck has 109k original miles, and when my family got it in '02, it had only 102k on it - You can tell it doesn't get driven much. I used to drive this truck when my car broke down a couple years ago, and even then it had some smoking carb issues; too much gas getting consumed.

I leave the state for a year or so, and I come back to the truck not operating worth a hoot, The smoking has turned from a mild white, to black. So god only knows what happened in that time, but I've noticed also on my intake manifold a bolt in the front of the ending is missing to it

so it's safe to assume that's the intake leak; also pumping the brakes also confirm a vacuum leak with the erratic idle.

Now, here's where I am totally lost . . .

After the car warms up completely (maybe 20 min of driving) ; slowing down for a light, or just putting the gear into neutral will shut off my car's engine even just shifting one gear to the other i try to do as quickly as possible because that half moment in neutral it wants to die.. It wants to drop completely when in neutral, and i've gotten very good at braking left footed while holding a tiny bit on the gas, let me tell you!

Everything goes back to being kinda fine after an hour or so cool down time for the car.

Say the car doesn't warm up all the way, i drive down the street, and then come back 10 minutes to the car say I stop at the grocery store for a pack of smokes, or a drink or something, my car will then want to shut off just as if it warmed up all the way.

I need to get pointed in some direction where to go with that, Like I said the truck doesn't have that many miles, so i'm concerned with what's happening here.

So you are saying you havn't driven it for a year? Are you in colder wheather during the winter months? You can have a lot of things going on with this vehicle, including break pads molding away, rotors, etc.

No, the brakes i've just changed --- I'm in florida, so the winters are non-existant (literally) --- I'm sure some of the seals throughout the engine have gone brittle because of non-use, but I haven't had the time (or effort/wanting to realize {lol}) where it's really at.
