I've got a 2003 Highlander. It didn't come with fog lights, but I'm thinking of adding them. The slots are there on the front, just covered over with plastic inserts.

All the equipment I've seen comes with a tacky aftermarket switch that you glue to your dash or whatnot... Is there a way to do this and look "factory"?

must buy OE stuff including the Light stalk mounted Fog lamp switch. Or go to a slavage yard. It's the best way to do this using OE stuff whether new or used.
